Patients with CDI had lower copeptin levels than patients with DH or SO (both P < 0.01) or those with NDI. Copeptin identified CDI with an AUC of 0.99 (95% CI 0.97–1.00), and a cut-off value ≤ 4.4pmol/L showed a sensitivity of 100% and a specificity of 99% to predict CDI. Indeed, recent studies have found promise in its use in managing sepsis as copeptin levels were found to distinguish normal controls from septic patients, and septic patients from septic shock patients [44]. It may also hold promise as a cardiology biomarker in conjunction with troponin for acute myocardial infarction rule out [45,46].

Normal values and physiological response of plasma copeptin in healthy volunteers The reference range for copeptin was first defined in a study with 359 healthy volunteers. This study showed the broad range of copeptin values. The median copeptin plasma concentration was 4.2 pmol/L with a range between 1 and 13.8 pmol/L.

1 In contrast, copeptin, a cleavage product of the AVP precursor protein, has high stability ex vivo and is released in equimolar concentrations, 1 with various studies establishing that copeptin levels accurately mirror AVP levels. 3,5,11,16 Measurement of copeptin is easily performed using manual or wholly

During arginine stimulation, clinical and laboratory parameters remained generally stable and in the normal range. Seven patients (all with diabetes insipidus) had mild hypernatraemia (range 146–149 mmol/L) at the end of the test. Overall, the test burden was low, although nausea was a frequent symptom during arginine stimulation. As a Control Group, 30 healthy individuals with normal physical examination results were included.

Median concentrations were considerably higher in men than women, increased significantly after exercise, and were influenced by fasting and water load. The normal range of plasma copeptin levels is from 1.0 to 13.8 pmol/l with higher median levels in men than in women according to two large physiological studies in healthy volunteers [26, 43]. The difference in gender is poorly understood, also because no significant changes during the menstrual cycle were observed [ 44 ]. Copeptin measurement has been shown to be useful in various clinical indications, including the diagnosis of diabetes insipidus and the monitoring of sepsis and cardiovascular diseases.

controls (median, 76.68 [Q1–Q3: 15.85–197.9 pg/ml]) and the difference was statistically significant (p < 0.01). Normal values of copeptin in healthy volunteers range between 1.70-11.25 pmol/L (6,9,10). Copeptin also shows minimal intra-individual variations (11). In healthy people, a 28h water deprivation leads to a twofold increase in concentration of copeptin, while infusion of hypotonic saline induces a 2-3-fold decrease of copeptin concentration (11). Copeptin levels doubled with each osmolality increase by 15 mOsm/kg (range, 10-21 mOsm/kg) (59%), from 9.8 pmol/L (range, 7.3-12.3 pmol/L) (48%) to 280 mOsm/kg. Copeptin kinetics differed between newly diagnosed and known T1D patients (P = .001), and less between mild vs moderate-severe DKA (P = .04).
In response to serum osmolality fluctuations, the kinetics of copeptin are comparable to those of vasopressin. No single reference range is applicable as copeptin varies according to plasma osmolality in normal individuals.
